O papel era muito exigente, exigindo que Mathew perdesse peso excessivamente. O ator estava perdendo peso de acordo com seu plano de dieta que incluía cerca de 1,5 quilos por semana até que ele conseguiu atingir a incrível perda de 22 quilos. O filme foi ambientado na década de 1980, época em que os procedimentos de tratamento do HIV não eram bem conhecidos e os pacientes passavam por um processo bem mais drástico de tratamento.
